What are you hours of operation?
- Monday: 8:00am – 4:30pm
- Tuesday: 7:30am – 5:30pm
- Wednesday: 8:00am – 4:30pm
- Thursday: 9:00am – 2:00pm
- Friday: 8:00am – 3:30pm
Are you accepting new patients?
Yes, we are currently accepting new patients. It is best that you check with your insurance to make sure Peak to Peak Family Medicine is in your plan’s network.

Complete the Medical Records Release form and submit it to the office either by mail, fax or in person. Please note the form must be completely filled out and the patient’s signature must be on the form.
How do I request a prescription refill?
Call the office and request a refill, be sure to know the name of your medication and understand we have a three-day refill policy to provide your pharmacy with the refill authorization.
What is a nurse practitioner?
A nurse practitioner is a registered nurse who has earned a master of science degree and has been certified to practice primary health care. Nurse practitioners provide direct patient care for routine exams, including well-baby exams and women’s health care; diagnose and treat common health problems; and order and interpret studies such as lab tests and x-rays. They are expert in the areas of prevention and guidance.
Do you perform labs in your office?
Yes, our medical assistant draws blood ordered by our providers and sends it to a lab for testing.
What hospitals are you affiliated with?
Our office and providers are not affiliated with any one specific hospital. It is best that you verify with your insurance which hospitals are in your plan’s network.
